Hotel Credit Card Authorization Form: Free Template and How to Use It

A hotel credit card authorization form is a document in which a cardholder approves a hotel to charge their card for a stay they may not attend themselves. Companies use it to pay for employees, parents use it for students, and groups use it for shared bills. Used correctly, it is the hotel’s main evidence in a dispute, which is why it belongs in any hotel chargeback protection plan.

When do hotels use a credit card authorization form?

Hotels use an authorization form in 4 situations:

  1. A third party pays for a guest’s stay.
  2. A corporate account pays for several rooms.
  3. A group master account covers shared charges.
  4. A phone booking needs signed approval for a large prepayment.

Direct online bookings rarely need one, because the booking engine records the guest’s acceptance automatically.

What should a hotel credit card authorization form include?

A hotel credit card authorization form should include 10 fields:

  • Hotel name, address, and contact details
  • Guest name and confirmation number
  • Arrival and departure dates
  • Charges covered: room and tax only, or room, tax, and incidentals
  • Maximum amount authorized
  • Cardholder name and billing address
  • Card type, number, and expiration date
  • Cardholder signature and date
  • The cancellation and no-show policy
  • A copy of the front of the card or photo ID, if the hotel requires it

Copy this template into your own letterhead and adjust the policy line:

Hotel: [Property name, address, phone]. Guest name: [ ]. Confirmation number: [ ]. Arrival: [ ]. Departure: [ ]. I authorize [Property name] to charge the card below for: [ ] room and tax [ ] incidentals, up to a total of $[ ]. Cardholder name: [ ]. Billing address: [ ]. Card type: [ ]. Card number: [ ]. Expiration: [ ]. I have read and accept the cancellation policy: [policy text]. Cardholder signature: [ ]. Date: [ ].

The template has no security code field on purpose, as explained below. Save it as a fillable PDF so cardholders type the details instead of handwriting them, which prevents declines from misread digits.

How do you fill out a hotel credit card authorization form?

To fill out a hotel credit card authorization form, the cardholder completes the guest and stay details, checks which charges are covered, sets a maximum amount, enters the card details, and signs. The hotel then confirms the details match the reservation before authorizing the card.

How should hotels store authorization forms?

Hotels should tokenize the card as soon as the form arrives and then destroy or redact the card number. PCI DSS forbids storing the card security code after authorization in any form. Paper forms in a drawer and emailed PDFs are the 2 most common PCI failures at hotels, as our hotel PCI compliance guide explains.

Is there a safer alternative?

Yes. A secure payment link replaces the form in most cases. The cardholder enters the card on a hosted page, accepts the terms, and the hotel receives a token instead of a card number. The hotel payment gateway handles both steps.

How long should a hotel keep an authorization form?

A hotel should keep the signed form, with the card number redacted, for at least 18 months after checkout. Visa and Mastercard allow most disputes up to 120 days after the transaction or the service date, and some disputes run longer. 3 rules keep retention safe:

  • Keep the signature, policy, and stay details.
  • Keep only the last 4 digits of the card.
  • Store forms in an access-controlled system, not a shared inbox.

An authorization form collects card details on paper or in a PDF, while a payment link collects them on a secure hosted page. A payment link records the cardholder’s acceptance, timestamp, and IP address automatically, so it is stronger evidence and keeps card numbers out of email entirely.

Does an authorization form stop chargebacks?

An authorization form does not stop chargebacks, but it wins them. A signed form with the policy, the folio, and proof the guest stayed answers most “not authorized” and “cancelled” disputes. Our hotel chargebacks guide covers the full evidence list.

Frequently asked questions

How do I get a credit card authorization form from a hotel?

Ask the hotel's reservations or front-desk team. Most hotels email a form or, more securely, a payment link for third-party billing, such as a company paying for an employee's stay.

Is a credit card authorization form PCI compliant?

It can be. The hotel must not keep the security code after authorization, must store the form securely or destroy it once the card is tokenized, and should prefer a secure payment link over emailed forms.

Can a hotel charge a card without the cardholder present?

Yes, with a signed authorization form or an accepted online payment. The form is the hotel's evidence that the cardholder approved the charge if they later dispute it.
